Forms in Stratus Online let you collect information from clients when they book, such as who’s coming, what they’re hoping for, or a signed policy. Answers go straight onto the client and session records in Stratus.
Before you begin: You’ll need at least one booking flow. See Bookings Set Up – Stratus Online.
Where a form can go #
| Placement | Best for |
|---|---|
| During the booking | Information you need before you can accept the booking. Add the form as its own page in your booking flow, and the answers are tied to that session. |
| Right after the booking | Intake and planning questions. Set the form as the Intake Form on the confirmation page, so it appears once the booking is already complete. It’s optional in the moment. If the client skips it, a personal link in your confirmation email lets them finish later on any device, right where they left off. |
| On its own | Forms that aren’t part of a booking, such as a lead inquiry form on your website. Each submission creates a new record in Stratus. |
Why use an intake form? With an intake form on the confirmation page, the booking is already complete, so a longer form won’t keep anyone from finishing their booking.
Creating a form #
Build forms in the Forms section of Stratus Online with the drag-and-drop form builder. You can also:
- Ask Asta to draft it. Describe the form you need, such as “a pre-session form that asks about allergies and the style they want, with a signature for our cancellation policy.” Asta builds a draft for you to review. Nothing goes live until you turn it on.
- Import forms from Stratus Desktop instead of rebuilding them.
Forms can show or hide questions based on earlier answers, collect signatures and file uploads, and let clients add family members or session participants.
Adding a form to a booking #
-
Go to Bookings > Flows and open your booking flow.
-
To ask questions during the booking, add a form page to the flow and choose your form.
-
To ask questions after the booking, open the confirmation page and set your form as the Intake Form.
-
Publish the flow.
Let clients finish the intake form later #
Add the intake form link to your booking confirmation email so clients who skip the form can come back to it.
-
Go to Content > Email Templates and open your booking confirmation email.
-
Add the Booking.IntakeFormLink merge field where you want the link to appear.
-
Save the template.
The link belongs to that one booking and doesn’t need a password. Not sure where something is? Ask Asta, “How do I add the intake form link to my confirmation email?”
Where the answers go #
Answers go directly onto the client and session records they belong to, and a copy of the completed form, including any signature, is attached to the record. If a form field is linked to information Stratus already has, the field is filled in with that information, and the client can update it if needed.
